On May 4 and 5, 2000, health care leaders, professionals, and academics convened at the Bechtel Conference Center at Stanford University for the Health Care Conference 2000. Sponsored by the Comparative Health Care Policy Research Project at the Asia/Pacific Research Center (Shorenstein APARC), in cooperation with the Center for Health Policy (CHP), the conference was held for the purpose of discussing health care policies and issues facing nations today. With the pressures of rising costs, aging populations in industrialized countries, and rapid technological advancements, the need for an accessible, affordable, and effective health care system is urgent and greater than ever. The first conference of its kind at Shorenstein APARC, the Health Care Conference 2000 established a forum for candid discussion about the past, present, and future of health care. Over sixty participants attended the conference. The panel consisted of speakers from governmental institutions, for-profit and nonprofit organizations, universities, and research institutes. The first day of the conference featured a discussion on the evolution of the health care market in the United States, while the second day focused on the effects of market forces overseas, specifically in England, Japan, the Netherlands, New Zealand, Scotland, and Singapore. The 1990s marked an era of major health care reform. For many nations with socialized health care systems, it was a decade to explore alternative systems and to move toward privatization. The implications of such changes were discussed in detail at the conference. The Health Care Conference 2000 was a successful and informative meeting, which opened the doors for future discussions on issues concerning health care around the world. These proceedings present, in edited form, the remarks of all primary conference speakers. His Excellency Sung Chul Yang, Ambassador of the Republic of Korea to the United States, is a well-known political scientist and author with a long and distinguished career in academia and politics. From 1996 to the time of his posting in Washington, Ambassador Yang served as a member of the Korean National Assembly. He also served as president of the Unification and Policy Forum, and chairman of the International Cooperation Committee for the National Congress for New Politics during this period. He worked as vice chair of the Unification and Foreign Affairs Committee, and was a member of the Political Reform Committee. Most recently, Ambassador Yang served as an executive member of the New Millennium Democratic Party's 21st Century National Affairs Advisory Committee. Outside of the National Assembly, Ambassador Yang has been involved in government and politics for many years. He served as the secretary-general of the Association of Korean Political Scientists in North America and as president of the Korean Association of International Studies. He has also been a member of the Advisory Committees of the Ministry of Foreign Affairs, the Ministry of National Defense, and the National Unification Board. In addition to his involvement in government, Ambassador Yang has had a successful career in academia. He was a professor at Eastern Kentucky University from 1970-75 and at the University of Kentucky from 1975-86. He has been a visiting professor at Northwestern University, Pembroke State University, Indiana University, and Seoul National University. From 1987-94 Ambassador Yang held the position of dean of Academic Affairs at the Graduate Institute of Peace Studies at Kyung Hee University in Seoul. Ambassador Yang is the author of several books on Korean issues, including The North and South Korean Political Systems: A Comparative Analysis (Westview, 1994). He is also a much sought after contributor to many political science journals. He has been interviewed often by leading newspapers, magazines and radio stations from around the world. Ambassador Yang received his doctorate in Political Science from the University of Kentucky (1970), earlier receiving an MA from the University of Hawaii (1967), and a BA from Seoul National University (1964). From 1960-62 he served in the Korean Army.

In Southeast Asia, partial recovery from financial crisis has been accompanied by signs of the start of a local version of America's internet business boom. E-businesses and dotcom start-ups have emerged. Political and business leaders have touted the virtues of the so-called "new economy" and its potential for resolving the post-crisis malaise that still affects parts of the region. But is the e-business revolution in Southeast Asia real? Are technologies and business practices being transformed? Or is the "new economy" mainly hype and wishful thinking? Does e-business spell the death of monopolies and conglomerates at the hands of agile new entrants? Or is it popular because it provides an alternative to fundamental business restructuring? And what do the answers to these questions imply for economic recovery and political reform? Linda Lim is the associate director of the International Institute and Director of the Southeast Asia Business Program at the University of Michigan in Ann Arbor. She has written and lectured extensively on political economy, economic development, and business practices in Southeast Asia, where she recently completed preliminary field research on the appearance and growth of electronic business.

Like a double-edged sword, the recent conflict in East Timor challenged Indonesia and the international community alike. For Indonesia, the crisis and its resolution offered a chance for military reform, yet threatened national unity. For international donors, the chance to defend human rights and implement self-determination carried a risk of provoking nationalist resentment against foreign intervention for democratic change.This talk will focus on the international community and its donor countries and agencies. How did they react to the conflict in East Timor? What were their strategies? How did their actions affect Indonesia -- not only its East Timor policies but also the course of its own democratic transition? Looking back on them now, do the crisis and its outcome lessons for the feasibility of foreign intervention to achieve domestic political reform? If so, what are they? If not, why not? And what do the answers to these questions imply for the democratic prospect in developing countries more generally?

David W. Brady is a political scientist whose work encompasses American politics and legislative bodies, international political trends, and comparative politics. Brady holds the Bowen H. and Janice Arthur McCoy endowed chair at the Graduate School of Business and is a professor of political science in Stanford University's School of Humanities and Sciences. A dedicated and popular teacher, Professor Brady is a past recipient of Stanford's Phi Beta Kappa Distinguished Teacher Award, presented for his work with undergraduates, and of the Robert K. Jaedicke Silver Apple Award, presented by the Stanford Business School Alumni Association for his participation in alumni activities.

Brady recently served as an associate dean for academic affairs at the Business School and continues to serve as director of the School's programs in executive education. He is a senior fellow at the Hoover Institution and senior fellow by courtesy at the Institute for International Studies, both on campus. David is also co-director of the University's Social Science History Institute and associate director of the University's Public Policy Program. He is a fellow of the American Academy of Arts and Sciences and member of the advisory council for the Kansai Silicon Valley Venture Forum.

Transcript of an address given by Richard Bush, chairman of the board and managing director of the American Institute in Taiwan, on May 24, 2000. Also included in this volume is the transcript of a roundtable discussion which took place on April 14, 2000, on Taiwan's historic elections. Three distinguished speakers participated: Larry Diamond and Ramon H. Myers, both senior fellows at Stanford University's Hoover Institution, and Suisheng Zhao, Campbell National fellow at Stanford's Hoover Institution.

This article explores an important but neglected topic in the literature on democratization in East Asia: the international dimension of democratization. It presents a coherent and comprehensive analysis of the impact of external political, economic and cultural factors on China, South Korea and Taiwan's political development since World War II. The author analyzes the circumstances under which the international context affects domestic actors' choice of political institutions and actions and concentrates on a selection of key international structures and actors that make up this complex picture. Shelley also examines the international political economy, aspects of the United Nations system, diffuse cultural factors and processes, democracy movements, and a number of international non-government organizations.

Panelist biographies: A specialist on democracy in Asia, Africa, and Latin America, Professor Diamond was in Taiwan as an official observer of the March election. He is co-editor of Journal of Democracy. A former associate editor of the Journal of Asian Studies, Dr. Myers has taught economics at National Taiwan University in Taipei. He was an observer of the March election. Formerly the Consul-General of Taiwan in South Africa, Dr. Feng is currently the Executive Secretary for the Research and Planning Board of the Taiwanese Ministry of Foreign Affairs. Dr. Zhao is author of Across the Taiwan Strait: Mainland China, Taiwan, and the 95-96 Crisis, and Associate Professor of Politics at Colby College. He was an official election observer for the Mainland Affairs Commission of Taiwan.

Among heads of state, Indonesia's is unique--and uniquely challenged. Aburrahman Wahid ("Gus Dur") is virtually blind and has suffered two strokes. His country, the world's fourth most populous, is bedeviled by political and economic crises. Momentum toward secession in several outlying regions, notably Aceh and Irian Jaya, threatens to unravel the republic. In Maluku province, intercommunal violence has accelerated. Prospects for continued recovery from the devastating recession of 1998 are uncertain. As impatience mounts over the inability of Gus Dur's government to date to solve such problems, fragile stability and nascent democratization are at risk of reversal, especially if elements in the army decide to intervene. Who is the man in the eye of this hurricane? What does his background tell us about his ability to cope with his country's manifold problems? Is he up to the task? Will he be ousted by his rivals? Or, in Indonesia's post-authoritarian political climate, do Gus Dur's weaknesses amount to a kind of strength? Donald K. Emmerson is a senior fellow at the Asia/Pacific Research Center. His edited book, Indonesia beyond Suharto, has just been published. Other recent writings include pieces in the Journal of Democracy and the National Interest--the former on political change in Southeast Asia, the latter on Western moralpolitik in East Timor. In June and August 1999 he helped to monitor Indonesia's national elections in Maluku and the UN's plebiscite on sovereignty in East Timor, respectively. His talk on Gus Dur will include information obtained during a three-hour interview with the new president in Jakarta in December 1999.

At Stanford, in addition to his work for the Southeast Asia Program and his affiliations with CDDRL and the Abbasi Program in Islamic Studies, Donald Emmerson has taught courses on Southeast Asia in East Asian Studies, International Policy Studies, and Political Science. He is active as an analyst of current policy issues involving Asia. In 2010 the National Bureau of Asian Research and the Woodrow Wilson International Center for Scholars awarded him a two-year Research Associateship given to “top scholars from across the United States” who “have successfully bridged the gap between the academy and policy.”

Emmerson’s research interests include Southeast Asia-China-US relations, the South China Sea, and the future of ASEAN. His publications, authored or edited, span more than a dozen books and monographs and some 200 articles, chapters, and shorter pieces.  Recent writings include The Deer and the Dragon: Southeast Asia and China in the 21st Century (ed., 2020); “‘No Sole Control’ in the South China Sea,” in Asia Policy  (2019); ASEAN @ 50, Southeast Asia @ Risk: What Should Be Done? Prior to moving to Stanford in 1999, Emmerson was a professor of political science at the University of Wisconsin-Madison, where he won a campus-wide teaching award. That same year he helped monitor voting in Indonesia and East Timor for the National Democratic Institute and the Carter Center. In the course of his career, he has taken part in numerous policy-related working groups focused on topics related to Southeast Asia; has testified before House and Senate committees on Asian affairs; and been a regular at gatherings such as the Asia Pacific Roundtable (Kuala Lumpur), the Bali Democracy Forum (Nusa Dua), and the Shangri-La Dialogue (Singapore). Places where he has held various visiting fellowships, including the Institute for Advanced Study and the Woodrow Wilson International Center for Scholars. 



Emmerson has a Ph.D. in political science from Yale and a BA in international affairs from Princeton. He is fluent in Indonesian, was fluent in French, and has lectured and written in both languages. He has lesser competence in Dutch, Javanese, and Russian. A former slam poet in English, he enjoys the spoken word and reads occasionally under a nom de plume with the Not Yet Dead Poets Society in Redwood City, CA. He and his wife Carolyn met in high school in Lebanon. They have two children. He was born in Tokyo, the son of U.S. Foreign Service Officer John K. Emmerson, who wrote the Japanese Thread among other books.

One of the most persistent and important questions in international comparisons of health systems pertains to the wide divergence in costs between countries. Japan has significantly lower per capita health care costs than does the United States, despite having a fee-for-service reimbursement system and universal coverage, and aggressively purchasing and utilizing equipment-embodied medical technologies. 1 One important factor in the increase in American health care costs over time has been the substitution of surgical intervention for medical treatment. 2 This leads us to consider differential rates of surgery as a potential explanation for divergent cost performances. Indeed, although Japan has one-half the inpatient admission rate of the United States, it has only one-quarter the surgery rate per capita
